Your first visit
We are a referral based office with a focus on providing specialty services under general anesthesia. Please see below referral form and have your primary care dentist/physician/regional center fill it out and sent to us! Your first visit will us will help determine the modality of care that best suits you or your loved one; it will help us identify if will be a suitable candidate here at our office. This will also help us triage your care in the event we are unable to provide the services you require.
New Patient Forms
Once your appointment is scheduled, you can pre-fill out your forms and have them ready for us! You can either email them to us at office@hilodent.org or bring them into your appointment on paper.
General Anesthesia
If you are deemed a good candidate to be seen in-office for general anesthesia, we will then initiate the process to obtain all the preliminary items required to facilitate the appointment. These items may include: funding/financial support, legal consents, preauthorizations, and a comprehensive medical history review. Please see below for pre/post op instructions for your general anesthesia visit.
